Western Pennsylvania Health Information Offers Communication Access & Accommodation Card

Daryl Dolan, in consultation with the Behavioral Health Task Force for Persons who are Deaf, Deafblind and Hard of Hearing, has created an accommodation card you can customize to give to health care providers and others. The card will give information on what you need to communicate effectively, who to contact for what you need, and information about people to contact in case of an emergency. There are separate cards for individuals who are deaf, hard of hearing, and deafblind.
